The Certified Ergonomic Evaluation Specialist Advanced (CEES-Advanced) recognizes a level of training and demonstrated ability in Ergonomic Evaluation as evidenced by completion of the Ergonomic Evaluation Certification Program, subsequent fieldwork, and submission of an electronic application packet (see link above) and sample of fieldwork. DO NOT SUBMIT REPORTS AND VIDEOS UNTIL REQUESTED TO DO SO BY THE CERTIFICATION COMMITTEE. When notified by the Matheson office, submit the reports/videos selected by the Certification Committee. These need to be electronically submitted via the instructions in the notification email.)
Disclosure
Certification lasts 5 years, but in order to continue to use the CEES-Advanced designation, the applicant must complete the recertification process before the expiration of the current certification, so please plan on beginning the recertification process after 4 years to allow time to attend the required course/s and reports.
Should the process not be completed by the end of the certification period, the applicant must either:
Attend the 4-day Ergonomic Evaluation Certification Program and then submit a an electronic application package including a list of 20 completed evaluations (do not send full reports or videos until asked)

Those Eligible for Certification
1. Those eligible for certification include active or fully qualified professionals working in the fields of Safety, Industrial Hygiene, or any one of the Allied Healthcare fields including, but not limited to, Nursing, Occupational Therapy, Physical Therapy, or Kinesiology.
2. Complete the Ergonomic Evaluation Certification Program.
3. Perform twenty ergonomic evaluations after the course is complete.
4. Submit a completed electronic application within 2 years of the date of completion of the course, which includes a LIST of the twenty evaluations performed by the candidate after the completion of the Matheson ergonomic course. (If application and evaluations are not completed within 2 years, the entire course must be taken again.)
5. Submit a non-refundable processing fee of US$200.00.
6. Successfully pass critique of up to three of the twenty evaluations.
The Application Process
Applications must be submitted (using the online application form) no later than two years after completion of the course. (Please refer to your course completion certificate for your course dates or call the office to request this information.) The applicant will be notified within 3 weeks regarding fieldwork submission and within 12 weeks of submission of the complete application and requested reports as to whether the applicants work has been accepted.
The application submitted for processing will include the following:
1. One Certification Application form
2. One Professional Experience form
3. One Statement of Professional Practice
4. Three Professional References (see Notes About Completing the Application below)
5. Listings of your most recent academic degrees
6. Case Study Submission form
Non-refundable fee of $200 (Check or Credit) – To be paid upon submission of the electronic application

Notes About Completing the Application
1. Statement of Professional Practice – Please read and consider this form carefully before signing it.
2. List three (3) Professional References that are individuals who have requested ergonomic evaluation services from you. Please inform these individuals that Matheson may contact them with questions regarding your evaluations.
3. When completing the Case Study Submission form, include the following information about the job task or position evaluated:
A The job or task title
B Whether task evaluated is classified as manual material handling (MMH) or upper extremity (UE)
C The date the evaluation was performed
Notes About Acceptable Evaluations
1. Ten of the twenty evaluations must be performed on manual material handling (MMH) tasks.
2. Ten of the twenty evaluations must be performed on tasks primarily related to upper extremity (UE) tasks.
3. Every evaluation must include the ANSI Z365 long form. (This will be requested to accompany your fieldwork upon notification of report selections.)
4. The NIOSH Lifting Formula form must be included when appropriate. (Reports that are missing the NIOSH form, when it should have been included, will be rejected. This will be requested to accompany your fieldwork upon notification of report selections)
5. At least 15 evaluations must have been videotaped. Indicate on your list which evaluations were NOT videotaped.
6. It is not a requirement that the evaluations be done in a professional work environment. Acceptable evaluations include, but are not limited to, the evaluation of tasks by friends or family members in the following environments: home office, routine housework, yard work, church or school events, etc.
Submitting Reports
1. DO NOT SUBMIT REPORTS UNTIL REQUESTED TO DO SO BY THE COMMITTEE. When notified by the Certification Committee, you will have SEVEN BUSINESS DAYS to submit the selected reports and videos ELECTRONICALLY according to the specific instructions within the notification email.
Processing of Application
The applicant will be notified within 12 weeks of submission of the complete application package and requested reports as to whether the applicants work has been accepted. If the applicant’s work is not accepted for certification, he/she will have 60 days from the date of notification to submit a new report which addresses the issues outlined in the critique.
Should the applicant’s resubmitted work not be accepted for certification the applicant must repeat the four-day certification program in its entirety at a cost of US$600 before the applicant can reapply for certification. A non-refundable fee of US$200.00 will be assessed for the new application for certification.
Future Recertification
The CEES-Advanced designation is valid for a period of 5 years from the date it is awarded. A date of expiration will appear on the certificate. It should be noted that the CEES-Advanced designation is a copyrighted designation; it may be used only by evaluators whose designation is current and in force. The right to use the designation expires at midnight on the last day of the month of the expiration month on the applicant’s certificate. In order to continue to use the CEES-Advanced designation, the applicant must complete the recertification process before the expiration of the current certification.

Special Exceptions
Questions about your eligibility for certification, request for deadline extensions and questions about report critiques must submitted in writing to the Certification Committee. The Certification Committee meets quarterly and will respond in writing to your questions or requests. (Please make every attempt to mail extension request letters at least 6 months before your 5-year deadline is up to ensure adequate time for response.)
